An important letter to all employees regarding 403(b) investment accounts can be found in the folder "Employee Benefits" on this page.

Please review three documents found in the "Employee Benefits" folder on this page:

1.) This identifies the open enrollment period and the various guidelines surrounding the change in health care carriers to SchoolCare. If you are planning to change coverages (ex. from POS to HMO), the new SchoolCare enrollment form should be submitted and received by Pam Dolloff by Monday, May 24th. If your coverage is going to remain the same, the transfer of information will be automatic and no form is required.

2.) 2010-2011 Health/Dental Insurance Rates - we have attached the new SchoolCare insurance premiums for the 2010-2011 school year. Refer to your respective bargaining agreement or contact the Human Resources Department to have questions answered.

3.) Declination Payment Form - If you are interested in receiving the declination payment, you must complete the Declination of Insurance Payment form, which can be found elsewhere on this page. Even if you received the Declination Payment previously, you must complete a new Declination of Insurance Payment form every year. The form must be completed and received by Pam Dolloff by June 30th. You must also provide proof of insurance from another carrier.

4.) Section 125 - If you plan to take advantage of the benefits offered with the Section 125 Plan, this form needs to be completed and received by Pam Dolloff by June 30th. This form must be completed every year.

5.) Dental Insurance - If you plan to enroll or change your current Dental Insurance coverage, the Delta Dental Insurance form needs to be completed and received by Pam Dolloff by June 30th.
